The transitional housing program in Passionate Behavioral Health Center, Inc. provides a program for men and women age 17 and older who are recovering from substance abuse challenges and suffer from homelessness. The transitional program provides the following services:
- A suitable, safe, and clean environment which is secure. This comprises a shared common area, kitchen, bedrooms, and bathrooms.
- It also provides a stable address where they can receive their emails as well as receive phone calls.
- Also, the program provides a case manager who assists the residents in achieving their short and long-term goals. The assistance includes helping them manage their addiction and mental health challenges, locating health care, building education and job training skills, and working towards having permanent housing.
- Residents could be in the program for up to 2 years, and they are evaluated on a quarterly and bi-monthly basis to see how they are doing with their treatment and recovery status.
A service coordinator from the transitional housing program will help set up and schedule services for this program.
